Call the office at (281) 578-5788 to schedule your initial patient visit and go over any questions you may have about joining the practice. For newborns, make sure to add them to your insurance provider ASAP, as they often only offer a 30-day coverage grace period. |

Click the Docusign link below to begin your new-patient paperwork. This may take 15-20 minutes. You can also print and complete the forms manually, if that is more convenient. |

Arrive 10 minutes early and bring your current health insurance card and valid driver's license (with current address that matches the insurance policy) with you. For newborns, please remember to bring all hospital discharge documentation to the first visit as well. |
